Three priorities after metastasis is identified
- Confirm pathology and molecular data: primary site, metastatic sites, histology, biomarkers and relevant genomic alterations.
- Reassess standard treatment: systemic therapy, radiation, surgery or other local treatment may be combined depending on tumor type, burden and performance status.
- Check genomic medicine and trials: eligibility depends on alteration, prior therapy, organ function and other criteria—not simply whether a treatment is new.
When a Japanese second opinion can add value
Examples include uncertain pathology, multiple reasonable treatment sequences, questions about local versus systemic treatment, or searching for genomic/trial options after standard therapy.
Cancer genomic medicine is not a guaranteed drug-finding test
Japan's National Cancer Center describes cancer genomic medicine as a form of individualized care based on tumor genetic information. Eligibility criteria apply, and a panel test may identify alterations without producing an immediately available therapy.
Where dendritic-cell and other immune-cell treatments fit
Japan has multiple registered treatment plans for NK-cell, T-cell and dendritic-cell approaches under the regenerative-medicine legal framework. Registration or self-pay availability is not the same as established standard-of-care efficacy for every cancer. Early-stage studies, including a Phase I neoantigen dendritic-cell vaccine study in recurrent solid tumors, are also ongoing.
Records to prepare
- Pathology report and information on slides/blocks
- Original CT/MRI/PET DICOM data
- Complete treatment history, doses, response and toxicity
- Genomic and biomarker results
- Recent blood tests and performance status
- Three to five precise questions for the Japanese specialist
